You can have the exam any where, as far as it's one of the authorized doctors.No need to update CIC. In the Instructions for Medical Exam letter, in the middle of the 1st page, highlighted in a box, it gives out the link of the list of CIC approved clinics outside of China. This means that you can carry out the exam at any clinic on this list. Hope it helps.
