I did it this october for my parents while I was visiting them. I submitted to visa center in shanghai on Oct 5th. Two days later, it was approved. I picked them up next day. Very quick.

no, i used the same forms. there are forms at visa center. Even you make some error on the form, you can always get another one. Staffs are very friendly and helpful.
