1. when you receive your H1b notice, you will find a new I-94 attached to the notice of action. it should be used with your old td i94. you will tell the boarder that you are coming to work.
2. for Canadian, you can get all H visas, TN/TD, B1/2 at the boarder instead of consulates. but even for Chinese PR in Canada, you will receive multiple entry H1b, there is no issue of visa application after the first time anyway.
3. there are 1 day passport service in Toronto, not sure if it is applicable to the first time passport application though.
