What is the difference between a Short Form and a Long Form?
A Short Form contains the following information:
Name (Surname and given name(s))
Date of birth
Certificate Number
Birthplace
Sex
Date of Registration
Registration number
Date issued
A Long Form is a certified copy of the birth registration and contains the following information:
Name (Surname and given name(s))
Date of birth
Birthplace (city, town, village, municipality)
Sex
Location of birth (hospital, birthing centre, etc.,)
Parent's names
Parent's date of birth
Mother's mailing address at the time of birth
Duration of the pregnancy
Number of children born to this mother
Parent's birthplace
Weight of the child at birth
Type of birth (single, multiple, etc.,)
Attendant at the birth
Date of Registration
Registration number
Date issued

Birth certificate for children under 16 years of age
For children whose parents were not married at the time of the child's birth, Passport Canada requires a long-form birth certificate showing both parents' information.
